Lightning VIP may use and disclose protected health information to provide, coordinate, or manage your care and related services, including referrals, lab review, follow-up, or consultation with other providers as appropriate.

For payment

Protected health information may be used and disclosed to obtain payment, process billing, support membership administration, or carry out related financial or administrative functions.

For healthcare operations

Protected health information may be used and disclosed for legitimate healthcare operations such as quality review, staff administration, licensing, compliance, credentialing, auditing, training, or business planning.

Appointment reminders and care communications

Lightning VIP may use contact information to communicate about appointments, follow-up, treatment-related information, care coordination, or other healthcare operations consistent with applicable law.
